Knowing-in-action that centres humanising relationships on stroke units: an appreciative action research study

Background:
Equal, collaborative and therapeutic relationships centred on the person affected by stroke are important for supporting recovery and adjustment. However, realising these relationships in hospital practice is challenging when there is increasing focus on biomedical needs and organisational pressures. Despite a body of evidence advocating for quality relationships, there remains limited research describing how to achieve this in clinical practice. This appreciative action research (AAR) study aimed to describe the processes involved in co-creating meaningful relationships on stroke units.
Design and methods:An AAR approach was used to develop humanising relationship-centred care (RCC) within two hospital stroke units. Participants were staff (n = 65), patients (n = 17) and relatives (n = 7). Data generation comprised of interviews, observations and discussion groups. Data were analysed collaboratively with participants using sense-making as part of the AAR cyclical process. Further in-depth analysis using immersion crystallisation confirmed and broadened the original themes.
Findings:All participants valued similar relational experiences around human connections to support existential well-being. The AAR process supported changes in self, and the culture on the stroke units, towards increased value being placed on human relationships. The processes supporting human connections in practice were: (i) sensitising to humanising relational knowing; (ii) valuing, reflecting and sharing relational experiences with others that co-created a relational discourse; and (iii) having the freedom to act, enabling human connections. The outcomes from this study build on existing lifeworld-led care theories through developing orientations for practice that support relational knowing and propose the development of RCC to include humanising values.

Introduction:
The factors associated with opioid poisoning death are poorly understood. We performed a retrospective autopsy study of decedents (a term used for people who are deceased) of opioid poisoning in Wales in 2015. Using anonymized linked data, we describe demographic characteristics, patterns of emergency service utilization, and clinical presentation prior to death.
Methods:Decedents of opioid poisoning in Wales in 2015 were identified from the Office of National Statistics (ONS) mortality dataset. Records were linked with the Emergency Department Dataset (EDDS) by the National Welsh Informatics Service (NWIS); and held in the Secure Anonymized Information Linkage (SAIL) databank. The data were accessed and analyzed in the SAIL gateway.
Results:Age at death ranged from eighteen to seventy-eight years, with a mean age of forty-two years. Average male age was forty-one years and average female age was forty-four and a half years. Seventy-three percent of decedents were men (n = 228/312). Eight-seven percent of decedents (n = 281/312) attended the emergency department in the three years prior to death. In total 2081 attendances were made, forty-one percent of which involved conveyance by ambulance. Attendances per individual ranged from one to 114, with over half of decedents attending more than three times. Diagnostic codes were mostly missing or non-specific, with only seven and a half percent of attendances representing eighty-two decedents, coded as drug related. Treatment codes were also mostly missing or non-specific, with sixteen percent of attendances representing 148 attendees attributed a treatment code. Thirty-nine percent of attendances (n = 822) ended in treatment and discharge, whilst twenty-seven percent (n = 562) led to hospital admission.
Conclusions:Matching previously published data, we found that fatal opioid poisoning is preceded by a period of high emergency health service utilization. On average decedents were in their fifth decade and more likely to be male than female. Attendances varied widely, with men less likely to attend than women.

U.S. land grant institutions currently follow? “top-down” model of research and extension in which farmers are the passive recipients of research results based on perceived needs which have been identified, prioritized, and investigated by scientists with little input from the end users. This exclusionary approach was designed to meet the needs of large scale, mono-cultural farming systems. However small scale, diversified, integrated systems are better served by a more participatory approach to research and extension in which farmers are actively involved as peers of researchers and extensionists in identifying research needs, designing and implementing experimental protocols, and delivering research results. This paper provides a brief outline of an alternative to the top-down approach termed “Farmers-First-and-Last”, developed and tested by Robert Chambers of the Institute of Developmental Studies, which has been effective in working with diversified farmers in developing countries.

On October 15, 1919, a press dispatch from Buffalo, New York, indicated that the primary election returns in that city gave an average of about three hundred votes to the three candidates whose names appeared on the ballot as representatives of the Communist party, the protagonist of the soviet form of government in the United States. The total number of votes cast was 54,000, which shows that the Communist vote was insignificant, numerically. Newspapers ridiculed the diminutive radical vote. All phenomena have a genesis, and it is the fact that a Communist ballot was cast rather than the quantitative character of the vote that has significance for the student of political philosophy and action. To those who saw the revolutionary left wing of the old Socialist party organize the Communist party and the Communist Labor party in Chicago during the first week of September, 1919, this initial appearance of the revolutionary Communists in American political life presages important developments. The event was heralded by revolutionists in this country and in Europe as the emergence of a new era in the political and economic life of the United States. The widespread dissemination of ultra-radical propaganda in connection with recent strikes is further evidence of the revolutionary purposes of Communists in America.
Optimistic prophecies are prevalent to the effect that bolshevism will find no fertile soil in the United States, since American workmen are too prosperous to become susceptible to revolutionary political and industrial philosophy. Similar predictions were voiced even on the floor of the Socialist emergency convention and the Communist Labor party convention last September.
